Client Form - Required
If you are new to our practice, or have recently moved and have a new address, please fill out and sign this form.
Note: due to state pharmacy regulations we like to have a client birthdate.
Please make sure that the Client Information Form is filled out completely and signed at the bottom.
Due to current California Pharmacy Regulations regarding certain common medications, we do need to have the client birthdate on file in your paperless record.
Patient Information Form - Required
Please fill out one of these forms for every new pet you are bringing in.
If your pet is boarding with us, please fill out this form and bring it with you to your pet's boarding drop-off appointment. NOTE: Please allow at least 15 minutes for your pet's boarding check-in. We want to make sure all of your pet's need are met!
Avian History Form - New bird cases
If you are bringing in a pet bird for the first time, please fill out the following form. It is a detailed history of your birds background and current lifestyle. This detailed information will help us to best evaluate your unique feathered friend.
